
1. Registration form must be filled in completely: Name, Social Security number, full mailing address, phone numbers, and date of birth.
2. For credit courses - fill in 7 digit course and section number, title, and credits. For community services courses - fill in course and section number and title.
3. Proof of pre-requisite must be included.

4. Fill in date last attended and program if applicable.
5. Sign form.
6. When registering by mail full tuition and fee payment must be made by filling out the credit card authorization at the bottom of the form.
7. Mail and fax registration is not available to students with veteran waivers, senior waivers, tuition waivers of any form, grants, loans, scholarships, and vouchers.
Please note: Incomplete registrations will not be processed
and will be returned to the students. |
